| Appointments
may be scheduled Monday through Friday 8:30 AM to 5:00 PM. Additional early morning appointments are available at 7:30 AM. If your condition
warrants immediate attention, please let us know and we will accommodate
you. If you are unable to keep your appointment or expect to be late, please
call our office as soon as possible. This courtesy allows us to better
serve other patients.
Spectacle eyeglass wearers
should bring all currently used eyeglasses and sunglasses with them to
the examination. Contact lens wearers are also urged to bring their most
current eyeglasses, as well as their present contact lenses. The examination
process may involve dilation of the pupils which increases light sensitivity.
Patients are therefore advised to bring a pair of sunglasses if possible
(temporary ones are available). Please anticipate that dilation of the
pupils will add an additional 30 minutes to examination duration. |

and Insurance Policies: |
| Professional
fees not covered by participating insurers are due in full at the time
of the initial visit. Eyeglass or contact lens purchases require a
minimum of a 50% deposit in order to place orders with our laboratories.
Any remaining balance must be paid in full at the time of dispensing of
ophthalmic materials.
We participate with
most local traditional insurance, H.M.O. and managed care networks. Ask
our staff if we participate with your plan. Please bring all current insurance
cards or applicable vouchers.
It is the patients
responsibility to pay any deductible amount, co-payment or any other fees
not paid by your insurer. |
